Providers taking City Call, will submit your online reporting form through this portal. Submissions come directly to the IPA administrator. All submissions should be sent by the 5th day of the month following the Pay for Call/City Call month. Should you require a record of your online reporting form, we suggest you “print screen” prior to submitting. For providers who are also submitting “Patient Contact Hours” (link to document) forms, please find that document under the Provider Resources tab, where you may print copies for your convenience. This document must be submitted by the fifth day of the month following the call month as well. Should you have any questions, please call the IPA office at 575-538-2355.
